Provena Mercy Medical Center is seeking a dynamic Occupational Therapist to join our team of compassionate, skilled therapists. At our facility, the Occupational Therapist is responsible for developing, organizing and conducting the occupational therapy program to facilitate rehabilitation. In this capacity, you will be responsible for directing patient participation and selective tasks to restore, reinforce and enhance performance; facilitate the learning of those skills and functions essentially for adaptation and productivity, diminish or correct pathology, and to promote and maintain good health. Additionally, you will be responsible for all required documentation in accordance to established standards. You will create treatment plans, which include a functional diagnosis, prognosis, goals and interventions, which will be developed for each patient/client. Opportunities are available for both the Inpatient and Outpatient setting. We have a great staff that is knowledgeable, helpful, and flexible. Our team works very well together and is seeking another strong team player. Specific Responsibilities: - Evaluation and assessment of the functional performance level as related to life skills of the individual patient
- Implementation and planning an occupational therapy program suited for the individual's goal attainment needs
- Maintenance of professional communication and working relationships with multidisciplinary and transdisciplinary teams to maintain and enhance performance of each patient
Requirements: Knowledge, skill and graduation from an American Occupational Therapy Association approved curriculum of Occupational Therapy with Bachelors degree or higher is required. To perform this job successfully, an individual should have knowledge of Word Processing and Meditech Order Entry software. CPR Certification and State of Illinois licensure as an Occupational Therapist are required. |
